There are significant updates and advances in the world of Interstitial lung disease (ILD) in diagnosis and management necessitating a nuanced approach to radiologic interpretation. This talk explores the pivotal role of thoracic radiology in distinguishing between various ILD subtypes, emphasising the contribution of high-resolution computed tomography (HRCT) in identifying key radiographic patterns. This lecture will revisit the imaging categorization of interstitial fibrosis; analyzing characteristic radiological features such as ground-glass opacities, reticular patterns, and honeycombing; interplaying with the functional, clinical input and advances of treatment, radiologists can provide more critical insights into differential diagnosis and disease progression. The clinical radiological correlation is further enhanced with discussion in multidisciplinary meetings to refine the diagnostic accuracy or consensus of diagnosis.
