A still under-utilised and unrecognised non-invasive cardiopulmonary exercise testing (CPET) offers a unique, opportunity to study both cardiopulmonary and metabolic changes concurrently. Broadly, CPET is able to distinguish between a normal and an abnormal exercise response and usually has the capability to identify multiple pathophysiological conditions alone or in combination which often leads to exercise intolerance. As such, it improves diagnostic accuracy and patient health care by pointing towards a more targeted diagnosis and treatment decision.

In general, CPET should be one of the early tests used to assess exercise intolerance. Having said this, this test requires specific knowledge and there is still a lack of knowledge for those physicians primarily interested in learning how to systematically analyse and interpret CPET results. This lecture encompasses the underlying principles of exercise physiology, cardiovascular, respiratory and metabolic responses.
