Extrapulmonary tuberculosis (EPTB) accounts for about 20–30% of all active TB cases. EPTB can occur through hematogenous, lymphatic, or localized bacillary dissemination. The diagnosis of EPTB usually involves clinical, radiological, histopathological and biochemical/immunological called composite reference as microbiological confirmation is difficult due to the paucibacillary nature of the mycobacterium. Early suspicion of RR/MDR EPTB is important in patients who are not responding well to the first-line anti-TB. The regime and duration of treatment of RR/MDR EPTB are the same as MDR PTB except for brain, spine and bone.
